 The
first question I ask myself is:What is the melody note?
The second question I ask myself is: What is the chord I am playing? The third question I ask myself is: Is this melody note in the chord? The fourth question I ask myself: What chord tones will I use to play below this melody note?  Now it will be valuable to point out here that, as you gain experience, this "questioning" process becomes so automatic that you know what you will be playing within a fraction of a fraction of a second!
